Could be the photo of Madonna was taken behind some ripply glass, somewhat like the kind used in glass shower curtains that help obscure whatever's in the shower. I suggest this because look at her hands, the effect is hardly apparent there, almost as if her hands are physically on the back of the glass, while the other parts of her body, likely a few inches back from the back of the glass are more rippled. The farther back the part of the body from the glass, the more rippled.
Or, they're using some sort of fancy gradient map and displacement filter.
My bets on some textured glass.
