Seems pretty obvious it was a mistake, why would they want to shoot down an airliner of civilians? The rebels previously shot down a Ukrainian plane, they tried again and fucked up.
Of course whenever something like this happens people will speculate that it's a false flag but it's not as if the Ukrainian government needs an excuse to go after the rebels.
Similarly the American government blaming Russia is silly, Putin clearly didn't want this to happen either.
