    Here's some recent ones:

    + Designers not giving accurate names to their layers

    + Project plans which don't include time for client review and re-touching

    + Asking for high-res images and getting low-res images instead

    + Getting images from a client only to find they are rights-managed and that those rights have expired

    + Flash developers who can't be bothered looking at XML or anything that's not FLA or AS beacuse it's "not in their department"
