Light is the most important thing I guess. Get the right light (mostly artificial or studio stuff), make the shoot and edit it in PS. Lots of work involved.

but mainly lighting.

In addition to the lightng.
Looks like the there might be a little wide angle lens action goin' on too. That gives it the semi distorted almost surreal feeling.

A relatively large and soft source, probably large box, that is overpowering the sun, probably by about 2 stops.

lens and lighting.
did someone already mention lighting?

beside lighting, they used lighting

actually, I don't think that is a softbox. And if it is, it's a small one. The shadows are really quite defined.
It looks like it's a single head (light) that's set to be about 1/2 to 1 stop brighter than the sunlight. The shadow of the guy on the ground is still pretty dense so I don't think it would be 2 stops.
And, it's almost certainly a wide-angle lens. Probably a 30 or something along those lines.
But, yeah, short answer is lighting. And, in this case the film or sensor setting that was used. The image itself is really saturated and contrasty. Which can be done in Pshop, but it will create noise if you overdo it.
The bottom line is: why do you ask? I don't think this is a very good photo at all.

who the h*ll knows, he/she could have done that a thousand different ways

Visia, agreed, probably a bare head close in. Although I'd still say maybe a stop or a stop and a half over.
