sucks...mistakes suck. with 3 people in your team and the biz dev team proofing it...
it really should have been avoided.
put a huge sign on top of your desk that says "PHONE NUMBERS!"
i have 4 of those signs and they read:
SPELLING
PHONE NUMBERS
CUTTING & FOLDING
FINAL PROOF*knock on wood
I haven't have any issues...

im the designer and to be honest im expected to make spelling and grammar mistakes. furthermore when you sit and stare at a page all day, you start to see through things.
its only reasonable to ask someone else to proof read it and if they do not do it (i ask for it to be proofed every time) then its thier fault.

an old boss of mine once decided he was perfectly capable of 'doing the translation' himself...english to french...on a Lever Ponds ad that was going to run in Quebec..
he doesn't speak a lick of french himself mind..but he'd done enough ads that he could wing it, he thought..
he thought wrong..
the ad went through the proofing stage, no one noticed anything..
until it actually ran..
and there were 10 fuckups in the translation...
big ones..
the sort that make french folk go nuts.
as in 'which dumbass anglophone thought it would be funny to run a french ad that was nothing but typos?
the french were insulted.heads rolled at Lever Ponds
young brand managers were fired.to this day, the old boss from that company has no idea why that senior brand manager at lever will not speak to him, nor give him new work.

I used to work at an agency were there were two proofers after me to look at every peice.. the only problem was they were both lazy ass punks... but the two or three times an ad or poster or whatever got to press with an error guess who got the shit for it... I guess the buck gets passed down and ends with the junior designer...

our whole i.m. team fucked up not too long ago with a mass email we did to over 100,000 persons.
the tagline was supposed to read something to the fact of "...as clowns would often do..." but instead as they typed it into word and sent it to us they typed in "...ass clowns would often do..."
no one saw it and the email was sent out on a friday. on monday all hell broke loose.
somehow everyone in i.m. missed the extra s. they came to me but i said 'all i do is copy and paste their text' and played the blame game (as you should).
