Employment Application Form

  So I have to redesign this "Application for Employment" form for a recruitment agency (UK / rail, construction etc.).

  Thing is - they only gave me a printed example (~20 pages)... they basically need an exact copy only with their logo + style..

  Now, should I

  a) scan -> recognize -> style
  b) google -> find similar -> style
  c) quit
  d) type-draw-style from scratch

  I know I'm lazy.
