You should be using Arial.
but seriously, could it be a font issue? maybe a corrupted font? Might be obvious but you have all the font settings right, correct? not using any alternates or anything?

this happened to me once on certain letter combinations, try turning on/off some of the opentype/ligature options in the type palette.

Thanks lads, turning off ligatures in all stylesheets seems to have done the trick. Not ideal mind you.
This seems to be a bigger problem with intel macs, here's an adobe thread in case anyone finds this thread looking for help:

That happened to me once with a Multiple Master font way back on OS8 or 9. I know that doesn't help. sorry.
