Nightclub industry = sleaze.
This does indeed seem like a two-fold issue...
01. There seemed to be a total lack of communication. You should have picked their brains about what exactly it was that they wanted. What were the goals here? Do you have proper documentation that they can fill out? Believe me, it helps.
02. Club owners/promoters. Notorious sleazebags.

why not try and set up a meeting with the next set of visuals and a load of printouts of all the other stuff you have done and talk it through face to face?

I always ask to the Nth degree whether a client is happy, whether they like X or Y, whether they've shown it around their office - and what the responses from colleagues or employees is, whether they had any deviation from the concept envisaged (this, after everything has been agreed anyway - if I can get them to admit they like something, then admit they'd prefer something else - well, it's extra billable hours, innit?).
This part due to being a quivering unsure wreck of a 'man', part due to clients being bound and gagged by some perverse social convention not to pipe up when they dislike something.
