Maybe this is obvious but I dunno.
If you're touching up a white background of a photo and want to be 100% sure it's white, use a threshold layer at 70% opacity.
Scan back and forward on the threshold layer to see blemishes on the white and use the dodge tool set to highlight 100% to clean those bits up. Keep scanning back and forward until it's all gone, you can quite confidently go close to your subject aswell usually like this.
This has worked well for me, even in room lighting, on an off white wall.
