Create an MS Word Template

I was wondering if anyone had a surefire way of creating a Template for word docs.... i.e inserting a header and footer with images in?
What format should I have the images in. eps/png/jpeg, Rgb/CMYK.
Any tips or advice?

I use indesign first to design the layout because word sux and is a pain in arse to use. Export as eps then open in photoshop save as rgb jpeg 180-300dpi (for printable doc) cut up as required at actual size. Insert header & footers or just full page header with 'pic behind text' option selected. I usually use tables in middle to space modifiable content as required.

I usually design the letterhead in Corel, export as a WMF and insert it as a background in Word. I've done the header footer route but if the client wants to use the page numbering, doc info, date features they always end up screwing things up. Also only using the header footer limits the layout to top and bottom.

Design in PS or AI then save as jpeg (rgb 300dpi).
Slice header and footer sections. Insert into word, making sure it is 100% in scale. Format picture: behind text, set picture position, allow overlap and lock anchor. Pull header and footer margins to desired positions. Hit save. At this point you will only have 5 seconds to escape before your computer emits poison gas and explodes, leaving a huge crater with your word template sitting at the bottom. Don't worry, in five minutes your client will call to make “minor” changes.

I would of assumed a PNG would be the best format but you guys think a JPEG?
Also, how would I create a WMF without Corel? I did not know ytou could drop in as a full BG image.

I have tried a number of ways now using different formats and still the PDF is fuzzy.
The only way I can see that works is to use and eps.
