basically you do this form with action='submitted.php' in submitted.php you include a script like phpToPDF this will populate your form inputs into a pdf file, could also be done with a less refresh manner by grabbing the GETS with ajax.

intVal is right.... an HTML form, possibly with jQuery validation post to a PHP script... and yes, some libraries help...

PrinceXML is your solution if you have some dough.
I use it on Cashboard to generate PDF invoices and estimates from customer HTML/CSS. It rocks. (http://getcashboard.com)
