Copy parts of photo to their own layers, filter/motion blur horizontal. Layer mask and blur the edges out.

This is one step in Photoshop 3D menu. Try 'create 3D from greyscale' or something like that in the 3D menu.
it will do exactly this.

Also seems to make use of the 'stylize > wind' filter, set to wind or blast.

remember to get a crappy stock photo image and filter it with dust&scratches or smartblur till the hair looks shit...
