12 Bit color filter or profile

I need to convert the colors used in an image to 12 bit depth (4096 color palette), as if I would watch the picture on an old rgb phone screen.
Background: I am playing around with a 32x32 LED matrix that is capable of showing 4096 colors and I try to let it display an animated png - and I want to setup the colors first.

You can use the Posterize adjustment in Photoshop to reduce it to 4096 colors very easily, but I don't know if that's true 12bit, or the exact palette of the LED matrix.

Set up an indexed palette then you have have it to those exact colours

Since my understanding of what you're after is 4bits/16 values.
