when designing for a perfect bound book, other than just not doing it at all...what's the best way of using a double page spread image and avoiding the gutter? 5mm blank margin on each page spine-edge?

I'd talk with your printer first. They will help guide you on how best to get both sides to register. How it's printed — press size, press type, folding machine tolerances, etc. — can matter a lot).

thanks. This is really a quick and dirty job, so just looking for a rough 'coverall' that 'will do'...it's for a test thing, not commercial, but it still needs to work in a dummy book way. The printer is a mass market online place so wouldn't enter into a discussion about machine tolerances etc...I agree though usually that's exactly what I'd do
