If you are up Pisa way then go to Lucca. It's a beautifu car-less town.
Some awesome beaches at Forte dei Marmi. Out of (italian) season these beaches are dead dead quiet.

Favorites are Arrezo, Sienna, Rome, Florence, Venice.
But out of all of them, Rome belittles you. The ruins and coliseum are magnificent. Just be sure to rotate your backpack to your chest on the train. Pick pocketers making bank there.

If you do end up in Florence, make sure you go and Michaelangelo's David at the Accademia -- it really is unbelievable.

We walked everywhere, a few times though we needed a car so we use Uber now that they are in Rome. This will prevent you from having to deal with a taxi that just makes up a price that usually isn't fair. Cause half the taxis will rip you off with non-metered prices.

From my girlfriend who's from Rome:
You need at least three days in Rome to see all the sites. Best way to go around is by moped, cars are only allowed in the centre of town at certain times, parking is expensive and difficult to find plus traffic is a nightmare. You can visit Naples in a day, have pizza at Da Michele, the best in the world, but go early (before 1 pm) as the queue is crazy. You might be able to see Pompeii and Ercolano in one day if you have an early start. You should spend a couple of days in Florence and same for Venice. I can give you contact details for a cool apartment in Venice. From there is much quicker to go to Milan than Rome for your inbound flight.

Yes, watch out for pick pockets. Have your passport either in the hotel safe or on a sleeve inside your shirt. I had some friends in a sleeper train gassed and all their passports, valuables taken. When on the train, just put your back back in front, not back. So you can manage the zippers.

Subway is easy and when I was there you could just walk in without paying (buses too). You're not supposed to, but everyone did it and no one stopped you.
Watch out for pickpockets though.

Pompeii was on strike when I was there. Literally. We took the train there, etc., just to learn they decided to close it (this happened numerous times on the trains, too, they'd just 'stop' for a few hours in the middle of nowhere!).
So we ended up going to Herculaneum, which was pretty cool too.
This was years ago, but just a heads up. Things don't run the same over there!

i think i'm gonna fly into rome, do rome stuffs, stay the night there then rent a car and head south to herculaneum and pompeii, stopping in naples for some legit pizza. stay in naples overnight after exploring the ruined cities. head back up to florence, stay and do shit there for a couple days, then go further north to venice, stay at a B&B there, then head west to milan to see the ossuary and bug georges before taking a train back to rome for my flight home. i've found a dozen or so non-touristy hidden gems i'll stop at along the way (grottoes, medical museums, relics, an ancient pharmacy)

I used these for a week. It was an amazing way to get around. Electrinic little cars do 45MPH and you can park anywhere. There have onboard ipdad navigation with all the best places to goto. And a great way to get noticed. people were taking photos of us everywhere we went. Turned more heads than a ferrari!

I wish I could help you out, but I'm the worst tourist ever, because I stay away from all the touristy places,
my advice if you want to really see rome, is to wake up around 6am and visit the most frequented places, we did with the lady some years ago and its amazing how beautiful the city is when there's no one around
